BCHP Children's Health & Safety Faire
BCHP would like you to be a part of the 10th Annual Children’s Health & Safety Fair on Thursday, May 10, 2007. Please complete a form and questionnaire and return no later than April 2 to Sutter North Medical Foundation, c/o Children's Health & Safety Fair, 969 Plumas Street Suite 207, Yuba City, CA 95991. If you are a fourth grade teacher in Yuba or Sutter County and interested in participating please completed School Participation Form. The Fair is first come first serve and filling up fast.
If you are a vendor who educates or in a profession to provide health education geared towards fourth grade students please complete the Participation Form. Space is limited and filling fast.
